STILL AND MOTION IMAGE RELEASE FORM

1. I grant permission for the use of my image, and/or likeness of it, to promote Salsa Santa Fe’s programs in any of its publications in print, digital, and/or online, without any form of payment or exchange of services, royalties, or other compensation at any time.

2. I waive the right to inspect the image and/or give approval of edits, changes, additions, copies, exhibitions, publications, or distributions of the image. I hold harmless, release and forever discharge Salsa Santa Fe from any claims, by myself or any one representing me and/or my estate, related to the use of my image.

3. I am over 21 years of age, and am competent to agree to the conditions of this contract. I have read this contract, understand its content, and agree to it, indicated by my signature below.
